Performs ONLY those functions specified for each individual consumer on the service plan.
Receives written instructions from the supervisor.
Has knowledge of Agency policies and procedures.
Is oriented and trained in all aspects of services to be provided to consumers.
If able to demonstrate competency in all areas of training for consumer services.
Assists persons at home in managing and maintaining a household, dressing, meal prep and incidental household tasks.
Will assist consumers with the following activities:
a.
Housekeeping b.
Eating and meal preparation c.
Arranging for transportation to and from appointments d.
Administering emergency first aid e.
Providing or arranging for social interaction f.
Providing transportation g.
Shopping h.
Companionship Reports any change in the consumer's mental or physical condition or in the home situation to his/her immediate supervisor or Agency Manager.
Completed consumer paperwork on the day service was provided and submits timely, per agency policy.

4 hour shift 8 hour shift Monday to Friday On call Weekends as needed Work Location:
In person.
Estimated Salary: $20 to $28 per hour based on qualifications.
